As a functional medicine practitioner, I’ve spent years helping clients uncover the root causes of their health challenges. One thing I’ve learned is that standard blood work often misses critical information that can make all the difference in understanding what’s really going on in your body.

Conventional medicine tends to look for disease markers rather than optimal wellness markers. This means your labs might come back “normal” even when you’re feeling far from it. In my practice, we dig deeper to find the subtle imbalances that conventional testing often overlooks.

Today, I want to share the top five blood tests that most conventional doctors don’t order, but that can provide invaluable insights into your health. These tests have helped countless clients in my practice identify hidden stressors, allowing me to create targeted healing protocols for them.

1. High-Sensitivity C-Reactive Protein (hs-CRP)

While standard CRP testing is becoming more common, the high-sensitivity version provides a much more detailed look at low-grade inflammation in the body. Conventional medicine typically only tests CRP when looking for acute inflammation or monitoring specific inflammatory conditions.

What makes hs-CRP so valuable is its ability to detect subtle inflammatory processes that might be flying under the radar. Chronic low-grade inflammation is at the root of almost every modern health challenge – from cardiovascular disease to autoimmunity, cognitive decline, and even depression.

An optimal hs-CRP level is below 1 mg/L. Anything between 1-3 mg/L indicates mild inflammation, while levels above 3 mg/L suggest significant inflammatory processes that need addressing. By catching this early, we can implement diet and lifestyle modifications before inflammation leads to disease.

2. Homocysteine

Homocysteine is an amino acid that, when elevated, acts as both an inflammatory marker and a methylation status indicator. Conventional medicine typically only checks homocysteine when there’s a family history of premature cardiovascular disease, but it tells us so much more.

Elevated homocysteine is associated with:

Most importantly, high homocysteine can indicate issues with methylation – a critical biochemical process that affects everything from mood and energy to detoxification and DNA repair. Optimal levels are generally between 5-7 μmol/L, significantly lower than conventional reference ranges that often extend up to 15 μmol/L.

3. Complete Thyroid Panel

The standard thyroid test in conventional medicine is typically just TSH (Thyroid Stimulating Hormone). Sometimes T4 (thyroxine) is included, but this gives us an incomplete picture of thyroid health.

A comprehensive thyroid panel should include:

  • TSH
  • Free T3
  • Free T4
  • Reverse T3
  • Thyroid antibodies (TPO and TG antibodies)

This complete picture allows us to identify subclinical thyroid conditions, autoimmune processes, and conversion issues that might be missed with standard testing. Many people suffering from fatigue, weight issues, hair loss, and mood disturbances have thyroid imbalances that don’t show up on basic tests but become evident with comprehensive testing.

In my practice, we look for optimal rather than normal ranges. For example, while conventional medicine considers a TSH between 0.5-5.0 mIU/L “normal,” most people feel their best when TSH is between 1.0-2.0 mIU/L.

4. Hemoglobin A1c (HbA1c)

While HbA1c is regularly tested in diabetic patients, it’s rarely ordered for individuals without diagnosed blood sugar issues. This is a missed opportunity, as HbA1c provides a valuable window into your glucose metabolism over the previous 2-3 months.

HbA1c measures the percentage of hemoglobin proteins in your blood that are coated with sugar (glycated). Unlike fasting glucose, which only gives you a snapshot of your blood sugar at a single moment in time, HbA1c reflects your average blood sugar over an extended period.

Conventional medicine typically only gets concerned when HbA1c exceeds 5.7% (the threshold for prediabetes) or 6.5% (the threshold for diabetes). However, research suggests that optimal metabolic health is associated with HbA1c levels below 5.3%. Even slight elevations within the “normal” range can indicate early insulin resistance and metabolic dysfunction that warrant dietary and lifestyle interventions.

Elevated HbA1c, even within the conventional normal range, has been associated with:

  • Increased cardiovascular disease risk
  • Accelerated aging
  • Cognitive decline
  • Compromised immune function
  • Increased cancer risk

5. Vitamin D (25-Hydroxyvitamin D)

While vitamin D testing has become more common, it’s still not routinely ordered for most patients. When it is tested, conventional reference ranges often suggest that levels as low as 30 ng/mL are adequate – but research indicates this is far from optimal.

Vitamin D isn’t just about bone health; it’s a powerful hormone that influences:

In functional medicine, we typically aim for vitamin D levels between 50-80 ng/mL (125-200 nmol/L) for optimal health. Achieving these levels often requires strategic supplementation and regular monitoring, especially for those living in northern climates.

Taking Control of Your Health

These five tests provide a window into critical aspects of your health that conventional testing might miss. If you’re experiencing unexplained symptoms or simply want to optimize your wellness, consider asking your healthcare provider about these tests.

Remember that “normal” lab values aren’t necessarily optimal. Functional medicine looks at narrower ranges that represent ideal health rather than the absence of disease. By identifying subtle imbalances before they develop into clinical conditions, you can take proactive steps toward genuine wellness.
